BEATRICE PUBLIC SCHOOLS HEALTH REQUIREMENTS

IMMUNIZATION REQUIREMENTS
ENTRY TO KINDERGARTEN OR SEVENTH GRADE:
- DTP (diptheria/tetanus/pertussis): 4 doses, with 4th given on or after 4th birthday
- Polio: 3 doses
- MMR: 2 doses
- Hepatitis B: 3 doses
- Chickenpox: 1 dose of vaccine OR proof of disease
STUDENTS ENTERING SYSTEM FROM OUT-OF-STATE.ALL GRADES:
- Same as above, except if child is age 13 or older they must have proof of having had chickenpox disease or 2 doses of chickenpox vaccine.
REQUIREMENTS FOR PHYSICAL EXAMS
- Physicals performed by a licensed health care provider within 6 months of admittance are required for entrance to Kindergarten and 7th grade. Such physicals are due upon enrollment. Physicals performed for HeadStart or other early education programs will be accepted for Kindergarten enrollment.
- Students entering the system from out of state must also submit a physical upon admittance.
- Sports physicals are required yearly for all sports grades 7-12 after May 1 prior to participation. The required 7th grade physicals will also serve as a sports physical for that year...no additional sports physical is necessary.
