Sports Physicals, Participant Consent and Emergency Contacts Forms
New each school year.
Before being able to practice or participate in athletics each season, the school needs paperwork filled out.
1. Proof of a current physical exam form signed by your/a doctor. Each May 1, starts a new year of "current" physicals.
2. A signed Parent & Student/Athlete Consent/Permission to Play agreement, with emergency information completed on the reverse side.
3. Pay a $53 activity fee per Student/Athlete. Collected only once a school year, limit of 2 collected per family. (Example: if you have 3 athletes who play a sport in each of our 3 seasons - fall, winter and spring - your family will pay only $106, but all will be eligible.)
4. A parent must attend one required parent meeting regarding sportsmanship and expectations each school year. Each year you must attend one meeting, but that will qualify your student/athlete for the whole year. (Example: you may have 3 athletes in all three seasons, but you will only have to attend the first meeting to take care of each of their requirements. You will likely hear the same cautions multiple years, but some of us need those reminders.)
